This work found that from September 2019 through September 2020 to the Ministry of health and Ministry of local government in collaboration with USAID Ingobyi team visited continuously (once a quarter)the 5 districts (Nyanza, Nyabihu, Muhanga, Gastibo and Nyaruguru) implementing HealthAppex for a verification session. Overall, DHMTs sustained improvement of functionality. In the past quarters, overall improvement in DHMT Functionality Score was slight in the majority of districts as a result of COVID-19 restrictions, which affected implementation of some planned activities. For the quarter four districts have numerously improved Nyauguru DHMT improved by 16.1 percentage-points from 77.4 to 93.5% and reversed Nyanza to its leading position since the last 4 quarters and it is the most improved DHMT. This was a result of their promotion of team work among its members and committed implementation of recommendations from previous verification sessions and promoting a spirit of innovation. Nyanza comes to the second position with score of 90.3.% from 87.1%; Nyabihu registered an increase of 10.3% and scored 80.6% from 70.9%; Gatsibo improved by 6%, from 67% to 74.1%.
